Amongst pleural tumors, primary tumors constitute a rare group. Those associated with paraneoplastic conditions represent an even rarer group, where metabolic disorders such as hypoglycemia could be observed, as is the case in Doege-Potter syndrome. We present the case of a 61- year- old woman with chronic progressive dyspnea with wasting symptoms and added vegetative symptoms associated with persistent hypoglycemia with hypoinsulinism and high IGF-2 levels, in whom there was evidence of a tumor of pleural origin in the left hemithorax. Anatomopathological findings described the mass as compatible with a solitary fibrous tumor, she underwent surgical resection, with subsequent resolution of hypoglycemia and recovery of respiratory symptoms. In the presence of an intrathoracic tumor associated with persistent hypoglycemia, it is necessary to rule out the possibility of Doege-Potter syndrome, which, if confirmed, has a high likelihood of remission with timely surgical treatment.
